Level 1.

The delivery of many existing services or pathways within health and social care is often dependent upon the skills and enthusiasm of individuals, rather than a systematic, repeatable process.

Level 2.

Services or pathways are planned, performed, measured and controlled but can be very different across health conditions, services and organisations.

Level 3.

A well understood consistent approach, integrated across health conditions, services and organisations and tailored to the needs of the individual.

Level 4.

Processes are controlled using statistical and other quantitative techniques supporting fact-based decision making within the service or organisation.

Level 5.

Processes are continually improved based on a quantitative understanding of the common caused of variation inherent in the processes across the service or organisation. This is an agile organisational state, where improvement is inherent in business and personal objectives.
